The "OK" is missing, unable to use the Supersized extension!

I installed the extension like any other extension however I tested the extension in using Dreamweaver 8 and DW CS4 the OK (submit) button is missing at the bottom of the window.

Please take a look at this screen shot;

I tried reinstalling for both versions of Dreamweaver; that did NOT fix the issue.

What is the screen resolution and the Text size options set in your windows settigns?

Seriously; any resolutions should work? Even if the resolutions is set very low; lets say 1024 x 768 or even lower you should still be able too see the okay button or scroll to see the button or resize the window to see the button; however you can NOT tab to the "OK" or even use a scrollbar to move down. (please take a look at my screenshot I provided, if you haven't already, in my original message)

BTW, I tried this using Dreamweaver 5 and the extension works fine; the okay button is showing fine.

This appears to be a major bug for older version of Dreamweaver users

the extension is tested and working fine in Lower DW versions.
Please provide me with your configuration, OS version, Screen resolution and font size options.
