mysql structure for DMXzone NivoSlider

How do you setup an MySql database to run in DMXzone Nivo Slider to run in a Dynamic system.I have folloed the istruction video which is not a problem
the only thing I do not understand is the database structure.
do I need to add bothe the image file name and directory
I can get the text to work OK but the image is blank
